Abstract

Aims to determine the bed requirement for a large district general hospital with a four‐bedded surgical high‐dependency unit (HDU). All admissions from 1 December 1997 to 28 February 1998 (aged over 15 years) were used in this study. Overall, the study showed that an estimated 1.6 per cent of adult, in‐patient admissions required HDU care (there were 10,949 admissions to the hospital during the study period). The bed occupancy for the surgical HDU during the study period was 90 per cent (319 actual bed days/356 potential bed days). The average length of stay was four days. The mean, median and modal daily bed requirement was six. The needs assessment study clearly demonstrated that it was essential for the Trust to provide more comprehensive care to a wider range of patients than was currently available in the hospital, particularly those with serious medical conditions, and recommended the provision of an eight‐bedded HDU.

Abstract

Purpose

Adipose tissue accumulation by trapping vitamin D and reducing its level may cause serious side effects. The purpose of this study is to determine the effects of vitamin D supplementation on dehydroepiandrosterone (DHEA), paraoxonase 1 (PON 1), insulin, free fatty acid (FFA), apolipoprotein-AI (Apo-AI) and apolipoprotein B (Apo-B) concentration in obese and overweight participants under low-calorie diet (LCD) program.

Design/methodology/approach

Healthy overweight and obese individuals (n = 70) with vitamin D deficiency were randomly assigned into 2 groups to receive either vitamin D supplements (an oral 2,000 IU vitamin D supplement) or placebo for 8 weeks.

Findings

All the participants were given an LCD program during the intervention. Vitamin D supplementation led to a significant increase in the levels of 25(OH)D (vitamin D vs placebo groups: 36.6 ± 9.8 vs 19.9 ± 3.5 ng/mL, p < 0.001), PON 1 levels (vitamin D vs placebo groups: 80 ± 25 vs 58 ± 23.2 ng/mL, p = 0.001), DHEA concentration (vitamin D vs placebo groups: 2.3 ± 0.7 vs 1.5 ± 0.6 ng/mL, p < 0.001) and Apo-AI levels (vitamin D vs placebo groups 3.7 ± 0.5 vs 3 ± 0.5 mg/dL, p < 0.001). Besides, intake of vitamin D supplements led to a significant decrease in FFA (vitamin D vs placebo groups: 3.1 ± 0.75 vs 3.5 ± 0.5 ng/mL, p = 0.001). After adjusting the analyses based on baseline levels, age and baseline body mass index measures, significant changes were observed in the insulin levels (0.03 ± 0.06 vs −1.7 ± 0.6 µIU/ml, p = 0.04). But the authors did not find any significant difference in the concentration of Apo-B between groups (vitamin D vs placebo groups: 71.5 ± 35.5 vs 66.6 ± 28.5 mg/dL, p = 0.05).

Originality/value

Overall vitamin D supplementation for eight weeks among vitamin D-deficient obese and overweight participants had beneficial effects on serum DHEA PON 1 FFA insulin and Apo- AI while it did not affect the Apo-B concentration.

Abstract

Purpose

This paper aims to investigate whether the interaction between vitamin D receptor (VDR) FokI polymorphism and dietary patterns is associated with metabolic syndrome (MetS) and its components.

Design/methodology/approach

In total, 304 Iranians were enrolled in this cross-sectional study. Dietary patterns were identified using factor analysis. Fasting serum glucose and lipid profile were also assessed. FokI polymorphism of the VDR gene was genotyped using the restriction fragment length polymorphism method.

Findings

Individuals in third tertile of “Unhealthy Patterns had greater odds for MetS (odds ratio: 2.9; 95 per cent CI: 1.3, 6.1; P for trend = 0.03) compared to those in first tertile. Significant results disappeared after controlling for covariates (p = 0.09). There was no significant relationship between adherence to ‘Healthy Pattern’ and odds of MetS (p = 0.55). There were not any interactions between FokI polymorphism and major dietary patterns associated with MetS.

Originality/value

No evidence found for the interaction between polymorphism FokI and major dietary patterns associated with MetS and its components in Iranian subjects. Genome-wide association techniques are needed to assess the direct effect of this polymorphism on MetS.

Abstract

Purpose

During the past few decades, production and marketing of functional foods has increased in many countries including Iran. Considering the fact that consumers’ preferences play an important role in the success of marketing a product to increase consumption, this study aims to fill the knowledge gap in this regard in Iran.

Design/methodology/approach

The theory of social marketing served as the framework of this study. Qualitative data were collected via eight semi-structured focus group discussions, between May and September 2014. Participants were 65 women (44 housewives and 21 employed women), aged 23-68 years, selected by purposeful sampling technique, considering maximum diversity. All focus group discussions were audio recorded and transcribed verbatim. Analysis of the qualitative content of the data was conducted using MAXQDA® software.

Findings

The findings showed that there were quite diverse preferences among studied women in regards to different aspects of a product and its social marketing strategies. The preferences toward functional dairy products were categorized in four main groups: characteristics of products including sensory and non-sensory characteristics; price; place of the product supply; and promotion strategies of products categorized in three subgroups of informing and educating, advertising and recommending.

Originality/value

This study is the first study in this regard in Iran. Given the novelty of functional dairy products in the market, the diversity of preferences should be considered both in production of dairy foods and their promotion plans. This understanding can contribute to success of interventions to increase consumption of these products among consumers.
